MENTAL RETARDATION SERVICES
- CENTRAL INTAKE & SUPPORTS COORDINATION
Services include: Intake, assessment, life management planning, psychiatric evaluation/consultation, treatment planning and service authorization. Case management services include monitoring, advocacy, service planning and continuity of care. Types of case management include: Early Intervention Casemanagement, Targeted Service Management and Waiver Casemanagement. In addition, services include CASSP program coordination, BHRS program coordination.
- Life Management Planning : individual support planning
- Casemanagement: Supports coordination except for Early Intervention
GUARDIANSHIP and REPRESENTATIVE PAYEE:
This service provides Guardianship for individuals and financial management assistance to individuals receiving Social Security Supplemental Income (SSI) and need assistance in handling their monthly checks.
RESIDENTIAL
This service refers to programs that assist individuals with Mental Retardation to reside in community homes with the supports they need. Residential services are intended for individuals to become independent in the areas of daily living, community awareness and social and personal development.
ADULT DAY PROGRAM
Community-based programs designed to facilitate the acquisition of basic skills such as: prevocational activities of daily living, independent living through the development of cognitive, physical and communicative abilities.
BEHAVIORAL SUPPORT SERVICES
The Focus Group provides support for individuals who are at risk of losing their community home, school or vocational opportunities. The Focus Group provides individualized supports by facilitating a team approach and the use of positive intervention strategies. This service is available to families and/or agencies that provide services for people with developmental disabilities. Referral is done through the Base Service Unit.
VOCATIONAL REHABILITATION
COMMUNITY EMPLOYMENT: Employment in community setting or employment-related programs which may combine vocational evaluation, vocational training and employment in a non-specialized setting. Included are competitive employment, supported/supportive employment and industry integrated vocational programs such as work stations in industry, transitional training, mobile work forces, enclaves, affirmative industries/business, placement and follow-up services.
FACILITY BASED VOCATIONAL REHABILITATION Programs designed to provide remunerative development and vocational training within a community-based, specialized facility (sheltered workshop) using work as the primary modality.
FAMILY SUPPORTIVE SERVICES
This service provides: 1) Supportive services designed to enable persons with mental retardation, and their families, to be maintained at home with minimal stress or disruption to the family unit; 2) Supports which enable the person to live independently in the community
